We want to make it easy for you to get the health care services you need. That’s why the UPMC for Life Complete Care (HMO D-SNP) plan offers trips to plan-approved locations at no additional cost.  

All members can schedule trips to medical-approved locations like the doctor’s office, the hospital, health care facilities, the pharmacy, and fitness centers.   

Members who qualify for Special Supplemental Benefits for the Chronically Ill (SSBCI) can also schedule trips to nonmedical locations like grocery stores, places of worship, salons, libraries, and community centers.* 

Using the transportation benefit is easy. You, a family member, or your caregiver can call to schedule your trip. If you need a wheelchair van or help to get into the vehicle, let the transportation service know when you call. Trips should be scheduled two weeks in advance and will be confirmed by 6 p.m. the evening before your trip.

*This benefit is part of the Special Supplemental Benefits for the Chronically Ill (SSBCI) program. To be eligible, you must qualify for Low-Income Subsidy (LIS) and have one of the following chronic illnesses: chronic and disabling mental health conditions, metabolic disorders, cardiovascular disorders, substance use disorders (alcohol, tobacco, opioid), diabetes, or other eligible conditions not listed. Eligibility for this benefit cannot be guaranteed based solely on your condition. All applicable eligibility requirements must be met before the benefit is provided. For details, please contact us.
