Enrolling in the UPMC for Life Complete Care (HMO D-SNP) plan is easy. Even if you’re with another health plan, you can easily enroll in our plan. Just follow the steps below.
How to enroll in UPMC for Life Complete Care
Follow the steps below to apply for enrollment:
- Make sure you qualify. This plan is available to anyone with both Medicare and Medical Assistance (Medicaid).
- Apply for enrollment. We offer three different ways to apply:
- Online: Apply online using our secure enrollment website.
- By phone: Call us toll-free at 1-844-575-8241 (TTY: 711) to apply for enrollment over the phone.
- Print: Complete, sign, and date the enrollment application and mail or fax it to us.
