How to enroll in UPMC for Life Complete Care

Follow the steps below to apply for enrollment:

1. Make sure you qualify. This plan is available to anyone with both Medicare and Medical Assistance (Medicaid).
2. Apply for enrollment. We offer two different ways to apply:
   • Apply online using our secure enrollment website.*
     
   • Call us toll-free at 1-844-575-8241 (TTY: 711) to apply for enrollment over the phone.
3. We will contact you once your enrollment application has been processed.
