The UPMC Health Plan HIPAA Fax Line 412-454-7829 has been permanently closed and is no longer receiving faxes. The most efficient way to submit a PRD (Personal Representative Designation) form is for the member to do so via the Member Site (Digital Account).

Completing a PRD Form on the Member Site is much faster and more efficient than sending in a paper PRD form; however, if the member needs to submit a paper request, please submit directly to:

When mailing HIPAA documents, please include the member’s name, DOB, and/or Member ID on each page of your document. This helps us ensure we have all the pages of a document, and that those documents will be added to the member’s account.
