Zero Copayment Benefit Limitations

The zero dollar copayment benefit applies to medical and pharmacy benefits and is available to members who reside in the Lehigh/Capital, Northeast, and Southeast Physical HealthChoices zones only. The counties included in these zones are Adams, Berks, Bradford, Bucks, Carbon, Centre, Chester, Clinton, Columbia, Cumberland, Delaware, Dauphin, Franklin, Fulton, Huntingdon, Juniata, Lackawanna, Lancaster, Lebanon, Lehigh, Luzerne, Lycoming, Mifflin, Monroe, Montgomery, Montour, Northampton, Northumberland, Perry, Philadelphia, Pike, Schuylkill, Snyder, Sullivan, Susquehanna, Tioga, Union, Wayne, Wyoming and York counties. Free Transportation Limitations

UPMC for You’s transportation benefit provides non-medical fixed route transportation and is available to members who reside in the Northeast and Southeast Physical HealthChoices zones only. The counties included in these zones are Bradford, Bucks, Carbon, Centre, Chester, Clinton, Columbia, Delaware, Juniata, Lackawanna, Luzerne, Lycoming, Mifflin, Monroe, Montgomery, Montour, Northumberland, Philadelphia, Pike, Schuylkill, Snyder, Sullivan, Susquehanna, Tioga, Union, Wayne, and Wyoming. The benefit is available at no additional cost. Mobile and paper ticketing options vary by county. If you are a UPMC for You member residing in the Northeast and Southeast Physical HealthChoices zones and have questions about how to utilize the benefit, please contact the UPMC for You Transportation Unit at 1-833-779-6827 (TTY: 711).
