Review your coverage
Whether you are a new or returning member, it’s important to take a few minutes to review your member guide. We update this guide each year, and the information it contains will help you take control of your health and your health care expenses.
Your Summary of Benefits lists the medical services that are covered under your plan. It also details the levels of coverage provided for those services. Before you seek care from any providers, review your Summary of Benefits for details about your copayments, coinsurance amounts, and out-of-pocket maximum.
Understand your prescription drug costs
If your plan includes Part D prescription drug coverage, it’s important to review your plan's formulary. This way, you can make sure your prescription drugs are covered and you understand your copays and cost sharing.
Setup autopay, if applicable
Paying your bill every month is easy with autopay. With this option, making recurring payments is a fast and secure process.
- In the UPMC Health Plan member site, navigate to: Spending and Billing > Premium Payments
- In the UPMC Health Plan app, navigate to: Account Hub > Manage Premium Payments
Select and schedule your annual wellness visit with your primary care provider
Make choosing or confirming your primary care provider (PCP) a priority. Your PCP’s goal is to help keep you healthy and give you the best possible care. PCPs offer no-cost preventive care—including annual wellness visits and vital screenings—and can coordinate any additional services you need. You can choose or update your PCP on the UPMC Health Plan member site or app.
Take your MyHealth Questionnaire
Completing this survey helps us better understand your health needs so we can provide you with the best health services possible. As always, you can also work with your doctor, a care manager, or health coach to create a plan for better health.
Download the UPMC Health Plan app
Our app makes it easy to navigate your insurance benefits, find the care you need, access virtual care with 24/7 video visits, and get answers to your questions through live chat. Get peace of mind from knowing your health insurance information is available on demand.
Update your communication preferences
Traditional mail could take days to arrive. Go paperless to receive your documents electronically and notifications by email or text message. You can update your preferences at any time with the UPMC Health Plan member site or app.
Review your care options
Getting the right care at the right time is important. As a UPMC for Life member, you have options to address an illness or injury. Knowing all your options can help you quickly get care in a cost-effective way. Select your plan type below to view your care options:
Review your recommended care
UPMC for Life covers most preventive services at 100 percent when you see a participating provider. Preventive screenings are important because they can help your health care provider detect diseases and conditions early, when they may be most treatable. Plus, you can earn rewards through the preventive care reward program for completing your annual wellness visit and other needed screenings.
Submit any necessary forms
- Prior Authorization form: Some medications may require prior authorization before our plan will cover them. If you had an authorization for a medication through your previous plan, a new prior authorization will need to be submitted through your UPMC for Life. The prior authorization can be initiated by your prescriber, or you can submit an authorization request through the UPMC Health Plan member site.
- Personal Representative Designation (PRD) form (PDF): This form lets you appoint another person to act on your behalf when discussing your health care coverage and benefit information. You can complete this form in the UPMC Health Plan member site or print the form and send it back to us.
- See more forms about your coverage and benefits on the UPMC for Life website.
